At Dungarvin, we help people live as independently as possible and thrive in their communities through personalized, person-centered case management. Our experienced case managers partner with individuals, families, and support teams to create plans that reflect each person’s goals, preferences, and needs, while navigating Minnesota’s service system.
We assist with accessing waiver and non-waiver programs, coordinating services, and connecting people to community resources. Dungarvin provides case management in Clay, Hennepin, Ramsey, and Wright counties.
Waiver programs include CADI, CAC, BI, DD, and EW. Non-waiver programs include Rule 185 Case Management, VA/DD Targeted Case Management (VA/DD-TCM), and Alternative Care (AC).
