Community-based supported living for adults with disabilities

Supported Living provides up to 24-hour community-based and home-based residential support for individuals who need structured environments and consistent staff presence to live safely.

Participants live in community homes or apartments with small group sizes, creating opportunities for friendships, shared routines, and individualized attention. Individuals may receive 24-hour support or scheduled daily assistance focused on personal care, daily living skills, health and wellness, medication support, behavioral support and access to community activities.

Staff also help people manage appointments, build routines, and stay connected to friends and family. The program promotes independence, stability, and a strong sense of belonging, while helping adults maintain meaningful roles in their neighborhoods.

Supports & Accommodations

24-Hour Support

This program model provides 24-hour caregiver support for individuals with intellectual or developmental disabilities.
Partial Support

For less than 24-hour needs, supported living services can be provided on a structured schedule.
Respite Support

Individuals receiving supported living services may stay in family homes when respite support is needed.
Living Environment

Participants live in community-based homes and apartments with support staff.
Small Groups

A maximum of four participants live in each community-based home.
Behavioral Services

Person-centered support for individuals with emotional and adaptive challenges.
Wellness services

Participants receive support for improving their physical and emotional health, promoting overall well-being.
Day Habilitation Community Supports

Day habilitation activities help participants build relationships and natural supports.
Ages Served

Services are tailored to adults age 18 and up, promoting independence and community life.
