A Day in the Life of a Dungarvin Case Manager

This important role ensures people receive the services they need

Dungarvin Case Managers assist people in obtaining the services they need to live as independently as possible and to thrive in their communities. They partner with individuals, families, support teams and county representatives to ensure the people they serve receive high-quality, person-centered support.

Alexandra King is a Senior Case Manager at Dungarvin Minnesota, where she coordinates Medicaid waiver services for individuals in need of supports. The waiver programs are a critical component of Medicaid, as they provide children and adults with home and community-based services (HCBS) as alternatives to nursing homes and other institutionalized settings.

Alexandra typically meets with an individual seeking services after the county has assessed their particular needs. Demonstrating Dungarvin’s person-centered approach, she talks with the individual and their family, or guardian, to determine the types of services that would best fit their unique circumstances, goals and desires. Leveraging her insight of these programs, she shares her knowledge of available resources and works with licensed service providers to establish supports for the individual.

Another important part of Alexandra’s role is ensuring that all necessary documentation is obtained and up to date for each of the 46 people she currently has in her caseload. She also works on support plans for persons served and submits service agreements to the county.

In addition to the paperwork, Alexandra meets with each individual she serves twice per year, whether in their family’s home, in their own apartment or house, or in a group home.

“Basically, we have a conversation and go over their current services, making sure the amount of hours of support they’re receiving is sufficient; and I ask if the service company is doing a good job and whether they need any services they’re not receiving,” Alexandra said.

As a Senior Case Manager at Dungarvin, Alexandra also provides knowledge and mentorship to others on her team. She meets weekly with a group of five to eight other Case Managers to answer their questions and provide additional insight.

Before joining Dungarvin four years ago, Alexandra had worked as an Inclusion Aide for a park department in Illinois. She helped children participate in park district sports and activities, as well as after-school programs. After moving to Minnesota, she worked for a nonprofit group home for adults with autism.

Alexandra enjoys working as a Dungarvin Senior Case Manager because it provides an opportunity to help others, and she’s always learning new things. “It’s fun to get to know the individuals served and other staff members,” she said, adding that she especially enjoys assisting people who are new to waiver programs. “It’s nice when we’ve figured out all of the logistics and they are set up with the services they need.”
