Supported Living provides up to 24-hour community-based and home-based residential support for individuals who need structured environments and consistent staff presence to live safely.
Participants live in community homes or apartments with small group sizes, creating opportunities for friendships, shared routines, and individualized attention. Staff assist with daily living skills, health and wellness, behavioral support, and community activities, with flexible schedules that can range from intermittent to around-the-clock care. The program promotes independence, stability, and a strong sense of belonging while helping adults maintain meaningful roles in their neighborhoods.

24-Hour Support
This program model provides 24-hour caregiver support for individuals with intellectual or developmental disabilities.

Partial Support
For less than 24-hour needs, supported living services can be provided on a structured schedule.

Individuals receiving supported living services may stay in family homes when respite support is needed.

Living Environment
Participants live in community-based homes and apartments with support staff.

A maximum of four participants live in each community-based home.
