Participant Assistance and Care (PAC) provides individualized health-related supports to help people live safely and independently in their own homes and communities. Staff assist with personal care, medication, mobility, and wellness activities while promoting autonomy, dignity, and choice in daily routines.

Support is scheduled around key times of day and tailored to each person’s needs, so seniors and adults receive reliable help without giving up independence. PAC services work alongside other home and community-based supports to reduce health risks, prevent hospitalizations, and help people remain where they are most comfortable.

Less than 24-Hour Support

Participants receive schedule care and health assistance.
In-Home Service

Health-related supports are provided in the participant’s own home.
Person-Centered Care

Participants get individualized attention for medical and daily living assistance based on their needs, preferences, and routines.
Ages Served

Services are tailored to seniors and adults age 18 and up who need medical assistance in daily life.
