Participant Assistance and Care (PAC) provides individualized health-related supports to help people live safely and independently in their own homes and communities. Staff assist with personal care, medication, mobility, and wellness activities while promoting autonomy, dignity, and choice in daily routines.
Support is scheduled around key times of day and tailored to each person’s needs, so seniors and adults receive reliable help without giving up independence. PAC services work alongside other home and community-based supports to reduce health risks, prevent hospitalizations, and help people remain where they are most comfortable.
