Mixed gender, currently supporting three individuals with diagnoses of mental illness, Huntington's chorea, TBI, depression, some with total care, all very pleasant and quiet, no aggression.  In-house/community activities include cards, educational TV, computer games, reading, knitting, shopping, restaurants.  Small quiet town, no public transit, Mall 30 minutes away.  24-hr staffing with asleep overnight.
N. Virginia   Open 11/20 Hennepin Co.      New Hope Yes- Includes accessible vehicle 245-B (DD) Huge rambler, five bedrooms, two baths, two living rooms, walkout basement (crisis room), large fenced backyard. Deck off main floor, ramp in garage. Large, newer accessible bathroom. Quiet neighborhood within blocks of public transportation, shopping centers.  Currently supporting two men, one woman 63-74. Presenting with Down Syndrome, demenita,  Alzheimers, severe MR, personality disorder. Level of care range from independent to full assist.  Available room is on main level, corner bedroom.  Staffing is 24/7 with substantial number of double staffing and awake overnight. 
Chowen     Moving to new home in 2010 Robbinsdale       Hennepin CO NO 245-B (DD) Split entry home in a quiet area close to Brookdale mall (I-94 and Hwy 100).Large backyard in an established older neighborhood. This home is in the process of changing the service model to supporting four female individuals in the transition age 17-27 presneing with Aspergers, Autism, FAS, mental health issues.  Home will be moving to a new location in  N.W suburbs in early 2010. Staffing will be double/triple depending the level of need.  Awake overnight.
Fair Place Sauk Rapids    Benton Co. NO 245-B (DD) This home is a four-bedroom rambler, large backyard, ramps in garage and bkyrd.  Currently serving two men, ages 37 and 47.  Both ambulatory, mild and moderated MR.  Verbal and no current issues with behavior.  Two bedrooms available.  One on the main and one in the lower level.  Mix gender is OK.  Staff is experienced with diabetic care. The Fair Place is a live-in model with primarily single staffing for both weekdays and weekends.  The overnight staffing is asleep.      
Osseo Henn. Co.    NO 245-B (DD) Large rambler located on a corner lot in a quiet neighborhood.  Four bedrooms on the main level.    3 ladies ages 40-65, functioning in the Severe to Profound range of developmental disabilities. There is staffing 24 hours per day, double staffing, and awake night staff.  The three women who live in the home enjoy community activities such as going to restaurants and church activities.  Each has medical issues but are ambulatory.  
France Pl. Brooklyn Ctr     Hennepin Co. No 245-B (DD) Rambler style house on a quiet residential street.  Three blocks from a small lake. Deck surrounded by a large fenced backyard.  France Place is ½ a mile away from Brookdale Mall. Currently supporting three ladies 40 to 58. One woman is deaf  using minimal sign language. Another is blind using adaptive equipment. Moderate to profound MR. Minimal aggressive behaviors. Asleep overnight.  Double staffing during the week for 4 hours each day for outings. The available bedroom on lower level with private bathroom/living area.
Zealand Henn. Co.      New Hope NO 245-B (DD) Large rambler with a ramp in front, homey and comfortable. Currently supporting three women 40's to 50's with few behavioral concerns, a fun group, getting along well with each other.  Three bedrooms on the main floor, one on the lower level. Grab bars in bathroom, long-term, stable staff. Some double staffing wkdays and wkends.  Staff is skilled in providing all ADL cares and seizure disorders. Would prefer a female, however, males will also be considered.  Overnight staffing is awake.
Eden Prairie    Eden Prairie    Hennepin Co. Yes, Includes accessible vehicle AFC (Corp. Foster Care) Duplexes.  The homes are located close to shopping malls, restaurants and movie theatres. Individuals served have complex medical needs requiring support with a wide range of physical and emotional needs. Each home serves four individuals. Each individual has his or her own bedroom. 1 ½  Program staff are available to each home 24 hrs per day. Full time LPN available on campus and RN support provided on a weekly basis.

The homes are located close to shopping malls; light Rail, restaurants and movie theatres. Individuals served have complex medical needs requiring support with a wide range of physical and emotional needs. Each home serves four individuals. Each individual has his or her own bedroom. 1½  Program staff is available to each home 24 hours per day. LPN, 40 hrs and RN support are also provided on a weekly basis.
 
Humboldt-     Brooklyn Yes         With roll-in shower Assisted Lvg.-Plus Humboldt is a newly built campus consisting of four one-level homes. Located on a huge wooded lot, targeted to serve 16 individuals.  Each home has four bedrms, two bathrms, kitchen/dining/living room, office/storage room.  Bus line within a block.  Property and services registered through MN Dept. of Health and operated under class-F license Assisted Living Plus.  Ideal candidate will need minimal assistance with ADL, is able to transfer independently, operate home appliances, arrange for transportation and handle financial matters with minimal support.  Two day and one overnight staff, campus supervisor is an RN.
