Elevators and stairs.  Underground parking available for additional fee.  Each individual is responsible for furnishing and providing for all the items necessary to live independently. If the individual has children they may live with them as well.  One outdoor swimming pool and laundry facilities on each floor. Lakeshore Oaks is slated to support 8-12 individuals of mix gender adults over the age of 21, maybe presenting with mental health and cognitive disabilities residing independently or with roommates.  Individuals must have skills necessary for  ADL's and cares and must be able to self-evacuate in case of emergency. Must be able to recognize their behaviors and de-escalate themselves.  The overall level of service will be based on the needs identified by individual and their team. Single staffing accessible 24/7 and additionally based on needs. Monitoring technology will supplement staff with overnight awake staff.  The availability of the apt is the responsibility of the apt complex's management, we can help facilitate that discussion. Dungarvin will work with individual and their team to set up tours and provide information regarding options available. For programming purposes staff provides transportation in their personal vehicles.

Lakeshore Oaks Apts - Crisis        With Technology       Mixed gender   Awake overnight Ramsey Co.     Shoreview    Harriet Ave NO 245-B (DD)      and CADI/TBI   Waivers        Not GRH eligible Crisis Apt Description- Dungarvin has a one-bedroom apartment within the Lakeshore Oaks complex which is associated with the Lakeshore Oaks apartment setting.  This apartment is available for crisis placement for individuals who are in transition with housing options, crisis stabilization, short-term placement or temporarily need a higher level of support in order to live independently at some point in the future.  The staffing pattern is developed to meet the needs of the person living in the apartment.  Placement is designed to be short-term until a permanent living arrangement is obtained.  For programming purposes staff provides transportation in their personal vehicles.

Four buildings each three stories tall with elevators and stairs.  Garages available for additional fee.  Each individual is responsible for furnishing and providing for all the items necessary to live independently.  Two swimming pools (indoor/outdoor), hot tub, tanning beds and a small workout room. Laundry facilities on site. Maj Cove is slated to support 12-18 individuals of mix gender, maybe presenting with mental health and cognitive disabilities residing independently or with roommates.  Individuals must have skills necessary for  ADL's and cares, but may require teaching opportunities to gain additional skills. The overall level of service will be based on the needs identified by individual and their team. Prioritization of the needs will determine the level of staffing and supports. Single staffing accessible at all times via technology, with overnight awake staff.  The ideal candidate would have at least five hrs of alone time at home as there is no staff on duty 9A-2P.  For programming purposes staff provides transportation in their personal vehicles.

Stone Grove is slated to support 14 individuals of mix gender, maybe presenting with mental health and cognitive disabilities residing independently or with roommates.  Individuals must have skills necessary for ADL's and cares, but may require teaching opportunities to gain additional skills. The overall level of service will be based on the needs identified by individual and their team. Extensive recreational activities offered by the apartment community. Prioritization of the needs will determine the level of staffing and supports. Single staffing accessible at all times via technology and awake overnight staff. The ideal candidate would have at least five hrs of alone time at home as there is no staff on duty 9A-2P. For programming purposes staff provides transportation in their personal vehicles.

Available bed on second floor. Thirteen steps. Single staffing and asleep overnight.

Individuals' level of care vary widely from fairly independent to needing assistance with ADL's. Moderate behaviors such as verbal aggression and some property destruction.  Very active in both in-home and community activities. Open bed is on main floor with hardwood flooring and close to accessible bathroom.  Double staffing almost all the time. Awake overnight staffing.

Currently supporting two women and one man ranging in age from 65-75. Individuals presenting with severe MR, seizure d/o, and mental heath concerns. Attention seeking behaviors including verbal and physical aggressions. Double staffing during most awake time when individuals are home, asleep overnight. House is wheelchair accessible with a ramp entrance, bathroom has a standard bath with grab bars.

Located on a huge wooded lot, targeted to serve 16 individuals.  Each home has four bedrooms, two bathrooms, kitchen/dining/living room, office/storage room.  Bus line within a block.  Property and services registered through MN Dept. of Health and operated under class-F license Assisted Living Plus.  Ideal candidate will need minimal assistance with ADL, is able to transfer independently, operate home appliances, arrange for transportation and handle financial matters with minimal support.  Two day and one overnight staff, campus supervisor is an RN.

 Hennepin Co.       Minneapolis Yes - Includes accessible vehicle CADI/TBI   Waivers Duplexes. The homes are located close to shopping malls; light Rail, restaurants and movie theatres. Individuals served have complex medical needs requiring support with a wide range of physical and emotional needs. Each home serves four individuals. Each individual has his or her own bedroom. 1½  Program staff is available to each home 24 hours per day. LPN, 40 hrs and RN support are also provided on a weekly basis.
Eden Prairie             Mixed gender Hennepin Co.   Eden Prairie Yes, Includes accessible vehicle CADI/TBI   Waivers Duplexes.  The homes are located close to shopping malls, restaurants and movie theatres. Individuals served have complex medical needs requiring support with a wide range of physical and emotional needs. Each home serves four individuals. Each individual has his or her own bedroom. 1 ½  Program staff are available to each home 24 hrs per day. Full time LPN available on campus and RN support provided on a weekly basis.
