Fully accessible, hardwood floors, close to shopping, churches and public transit.  Currently supporting two males and one female ages 20-80, presenting with MR, paraplegia, cerebral palsy, dementia, and ADHD.  Some attention seeking behaviors, hyperactivity and loud vocalizations.  Moderately active in community.  24-hour awake, double staffing most hours.
Ballantyne Spring Lake      Park        Anoka CO. Yes         Ramps, no roll-in shower 245-B (DD) Two story home, in older, quiet, very nice neighborhood, big trees, fenced backyard, accessible ramps in front and backyards. Hoyer lifts and grab bars in bathroom.   Currently supporting two females and one male ages 30's and 40's, borderline personality disorder, non-verbal, unsteady gait, some verbal aggression and property destruction.  Very active in the community.  Home is close to Northtown Mall, shops and restaurants.  Open bed on main level, it is accessible and carpeted.  Substantial number of double and some triple staffing.  Awake overnight staff. 
Xerxes Bloomington   Hennepin NO 245-B (DD)  A rambler with two bedrooms/bathrooms on the main floor and two in the basement. Currently supporting three women ages 25-32 severe to mild MR, mental health issues and Autism.  Female roommate is preferred.  Very active in the community, shopping, movies, dining out, dances, crafts, table games, music, and TV.   Occasional verbal outbursts or aggression.  Substantial number of double staffing on wkdays and wkends.    Asleep overnight staffing.  Available room is large and in the lower level requiring using a full flight of steps. Large, fenced backyard and deck.  Many community resources, dedicated house van. staff is experienced in psych med monitoring, diabetes, autism, high medical needs, and mental health issues.      
Elm Creek Maple Grove NO 245-B (DD) Split Level Home, situated in a residential area near Fish Lake with several nearby parks.  Providing supports to transition-age young males. The current age range being 17-21, attendding Maple Grove high school through the Osseo School District, various day programs, have jobs or are in transition. Very active home with activities agreeable to all and based on individual needs. Diagnoses include mod/mild MR; ADD/ADHD; Aspergers, FAS, as well as mental health disorders. Some individuals may be involved with or have been exposed to the legal system. The home has significant double staffing; awake-night supervision and day time supervision as needed.  The home has 2 bedrooms, 1 bathroom and 1 living room on each level, elevated deck off the dining room, not handicapped accessible.  Training of staff includes behavior management, crisis prevention, mental health as well as other training corresponding to the needs of the home.
Fair Place Sauk Rapids    Benton Co. NO 245-B (DD) This home is a four-bedroom rambler.  Currently serving two men, ages 37 and 47.  Both ambulatory, mild and moderated MR.  Verbal and no current issues with behavior.  Two bedrooms available.  One on the main and one in the lower level.  Mix gender is OK.  Staff is experienced with diabetic care. The Fair Place is a live-in model with primarily single staffing for both weekdays and weekends.  The overnight staffing is asleep.      
France Pl. Brooklyn Ctr     Hennepin Co. NO 245-B (DD) Rambler style house, quiet street, three blocks away from a small lake. Deck surrounded by a large fenced backyard.  Home is ½ a mile from Brookdale Mall and many community amenities. Currently supporting three ladies moderate to profound MR, ages 24-58. One of the ladies is deaf using minimal sign language. Another is blind, using adaptive equipment. Minimal aggressive behaviors. Asleep overnight.  Double staffing wkdays 4 hrs at primetime. Available room on the lower level with its own private bathroom and living area.
Norway Foley         Benton Co. Yes AFC (Corp. Foster Care) Beautiful 4-yr old one-level home, vaulted ceiling, situated on a marsh/natural preserve, 100% accessible with roll-in shower, grab bars. Mixed gender, currently supporting three individuals with diagnoses of mental illness, Huntington's chorea, TBI, depression, some with total care, all very pleasant and quiet, no aggression.  In-house/community activities include cards, educational TV, computer games, reading, knitting, shopping, restaurants.  Small quiet town, no public transit, Mall 30 minutes away.  24-hr staffing with asleep overnight.
Banaher Crystal NO 245-B (DD) One-story rambler located in a quiet older, established neighborhood.  All four bedrooms on the main level, partially finished basement.  Currently supporting three men ages 45-55, non-verbal,  presenting with profound to severe range of MR, down syndrome, blindness, autism, needing help with all personal cares.  Some SIB, sensitivity to noise.  24-hr awake staff, two of the individuals are retired.

Minneapolis     Hennepin Co. Yes - Includes accessible vehicle AFC (Corp. Foster Care) Duplexes. The homes are located close to shopping malls; light Rail, restaurants and movie theatres. Individuals served have complex medical needs requiring support with a wide range of physical and emotional needs. Each home serves four individuals. Each individual has his or her own bedroom. 1½  Program staff is available to each home 24 hours per day. LPN, 40 hrs and RN support are also provided on a weekly basis.
Eden Prairie   Touring for waiting list Eden Prairie    Hennepin Co. Yes, Includes accessible vehicle AFC (Corp. Foster Care) Duplexes.  The homes are located close to shopping malls, restaurants and movie theatres. Individuals served have complex medical needs requiring support with a wide range of physical and emotional needs. Each home serves four individuals. Each individual has his or her own bedroom. 1 ½  Program staff are available to each home 24 hrs per day. Full time LPN available on campus and RN support provided on a weekly basis.
Turtle Ln. Shoreview   Ramsey Co. Yes         With roll-in shower 245-B (DD) Large rambler, big fenced backyard, a gazebo in a very nice quiet neighborhood, close to shopping.  Currently supporting three older individuals one male (non-verbal), two females (one is nonverbal) ages 50-70, one is retired.  Profound/severe MR, Alzheimer's, moderately active in community.  24-hr staffing with asleep overnight, double staffing based on needs.  Turtle Ln. is a good home for people with accessibility needs.

Located on a huge wooded lot, targeted to serve 16 individuals.  Each home has four bedrms, two bathrms, kitchen/dining/living room, office/storage room.  Busline within a block.  Property and services registered through MN Dept. of Health and operated under class-F license Assisted Living Plus.  Ideal candidate will need minimal assistance with ADL, is able to transfer independently, operate home appliances, arrange for transportation and handle financial matters with minimal support.  Two day and one overnight staff, campus supervisor is an RN.
