Dungarvin Minnesota believes a person's home should be a place
of his or her choosing. We will provide the assistance and
support necessary to create a healthy and happy atmosphere. This
may be in a rented apartment or house, or a home owned by the
people receiving services. Individuals may live alone or with
others. Typically, four people live in most sites.
Services are also provided to people living with parents or
foster care providers.
You will not see a difference in appearance of the homes where
we provide services and the other homes or apartments in the
neighborhood. We believe they should be clean, safe, and
attractive.
Getting Started
To
seek services, you will need a case manager, service
coordinator or social worker. Contact your county information
desk and ask for assistance from the community services for the
department serving persons with disabilities.
The case manager will provide you assistance on obtaining a
funding method. The most common funding source for our services is
the Home and Community Based Waiver program. However, there are
a variety of other funding sources as well. Once you have a
waiver, your case manager will assist you in looking for
appropriate services based on your needs.

In-home Services
In-Home Services are
developed to serve individuals in their own living environment.
We serve individuals with a broad range of abilities and
needs. Your child or family member may require supervision
for their daily living routines or assistance with accessing
community resources. Your child or family member is monitored
and supervised to the extent requested by the team. Our in-home
services vary. We may go into your home to assist your family
member with managing their finances. We may take them out
weekly for swimming, grocery shopping, community education
classes, or social events. In-Home respite care is also
available. We can provide in-home services in any county we
contract with throughout the state.

Supported Apartment Services
Supported Apartment Services is an
alternative to traditional adult foster care that allows
flexibility to provide services tailored to the individual. These
services are designed to adapt to the increasing or decreasing
needs of individuals who live in their own apartments. Services
available include 24-hour emergency staff response with technology
and supplemental support in areas including budgeting, menu
planning, grocery shopping, assistance with medical appointments,
and additional areas as identified. Supported Apartment Services
are currently located in the northern and southern Twin Cities
areas with additional locations being developed throughout the
metropolitan area and outstate.

Supported Living Services (Residential
Group Homes)
These programs are licensed as adult or child foster care or as
a supervised living facility by the Minnesota Department of
Health, and have a program or service license from the Minnesota
Department of Human Services under 245B.
Dungarvin Minnesota operates nearly sixty SLS sites throughout
the Twin Cities metropolitan, Hutchinson, and St. Cloud areas.
Each site serves two to six individuals. These sites serve
individuals of various need levels, including individuals with
significant medical, physical, or behavioral challenges.
Staffing levels vary by the needs of the people being served.
Adult
Foster Care Services
Wheelchair
accessible homes serving individuals on the CADI/TBI/CAC
Waivers and private pay. Individuals served have
complex medical needs requiring support with a wide range of
physical and emotional needs. Each home serves four
individuals.

Independent Living Services
ILS services are very
similar to the In-Home Services described above. The most
significant differences are that this is not
available to those on the MR/RC waiver. It is for those
people on the TBI, CAC, or CADI waivers. The other
difference is that there is a stronger focus on learning or
maintaining independence skills.
Assisted Living Plus Services
The newly built Humboldt campus is
targeted to serve 16 individuals in four homes. It supports
individuals on the CADI/TBI/Elderly waivers and private pay.
The property and services are registered through the MN Dept. of
Health and are wheelchair accessible.
Transitional (Temporary) Services
Dungarvin provides
short-term transitional placements in various group homes.
This service is typically for individuals who are in a crisis
situation and need a place to live temporarily. We can
serve individuals in a currently vacant placement, or as an addition to
the current individuals being served in a site. We can
provide 24 hour services to people who have challenging
behaviors, medical needs, or physical disabilities. Most
services are for 90 - 180 days.
Respite
Services
Dungarvin can provide
respite services in any of our group homes that currently have
an open bed. We can provide services for as short as one
day. Typically, services are no longer than 14 days.
We may be able to advance book a placement or have a standing
repeated placement. We are only able to provide respite to
adults at this time.
