regularly scheduled to work 30+ hours a week may enroll in medical
benefits the first of the month following two full calendar months
of employment in the benefit eligible category.

care (i.e. teeth cleanings and routine x-rays) covered at 100% of
usual and customary cost

months in plan, basic services such as crowns and root canals are

are covered at 50%
